Thank you page 404 error

Hi,
Was testing the contact us form (http://www.praisewear.com/mm5/merchant.mvc?Screen=CTUS) on the ReadyTheme and after submitting, I am getting this:

http://www.praisewear.com/THANKYOU.html?form=contact

The form does work and sends to my assigned email, but the customer will get a not found page and it appears it didn't work.

Any ideas on where to get the thank you page working?

The form submits to a page called THANKYOU and as it looks this page doesn't exist. Either create the page or submit to the home page or another page by changing the form submit code (screen=THANKYOU).

Hey - thanks for the help!
I created and uploaded THANKYOU.html, but for some reason, when I submit via the contact form, its not finding it. Here it is: http://www.praisewear.com/mm5/THANKYOU.html

I tried putting the html file in the main directory of my site, inside the mm5 directory and inside the mm5/forms directory, but it won't find it.

You have to create it in Miva under user interface > pages and the URL is without the /mm5 http://www.praisewear.com/THANKYOU.html.When creating the page make sure to assign all required items.
