That produces a Fatal Error (looks like the form within a form error)

Yes, try changing that to

 data-action="&mvte:urls:CTFM:auto_sep;Store_Code=&mvte:global:Store_Code;"

Not to the data-hook, but the data-action.

The form has this

data-action="&mvte:urls:CTFM:auto;"

It does include the store code in it (when viewing the source code for the page). Is that what you are referring to?

Matt Zimmermann - how do you make this new form work with a Mall setting? There seems to be an issue with the data hook.

The only thing I can think of is that your product form is missing the attribute name="add".

This got figured out - not all of the CSS files made it to the new "combined" file.
