Looking for MS Outlook alternative - with Custom Forms

Hi, I'm looking for a contact manager program that allows you to create custom contact forms, similar to Outlook's custom forms. I would continue using Outlook, but Outlook 2016 doesn't support attaching journal entries and tasks to contacts. I need that abilitiy as well.

Any ideas would be greatly appreciated. Thanks!

I use Nimble (nimble.com) for creating canonical contact records. I may allow you to do what you are looking for, and it has the ability to automatically draw in social profiles and data for contacts as well.
