USPS insurance is based on declared value and has a limit of $5k.
It' a small table. Like less than 10 lines. Some of the non-std options already have insurance included.
Bill had a module that did insurance by subtotal which would accommodate exactly what you need to do, but I don't remember whether it was universal or could be selected.
Anyway, USPS insurance is pretty cheap. Being universal may not be a bad thing.
Our USPS Advanced module does provide the ability to offer shoppers an additional insurance coverage option. It can be a forced charge or optional. For example Priority Mail would be presented as two options -
Priority Mail ($15.75) Priority Mail ($15.75) + Insurance ($4.75) ($20.50)
