Additional customer information for checkout

I'm selling a report between two people. I have to have the names and birthdates for both people for the three different types of reports.
In addition, I'm giving discounts for multiple reports, up to three. So I need a place to put the type of report they want, both sets of names and birthdates for all three reports.

In other words, I need to put a place for six names and birthdates.

Does anyone know which module I need that would do this?

You cannot do this with MIVA Merchant Express.

It does not allow any third party modules. You would need to upgrade to the full version of MIVA Merchant 5.
