PCINET>Map Locations with Miva 9 (modified Levels theme)

Has anyone purchased and installed PCINETs Map Locations successfully with a Miva 9 store? I got version 5 and the upgrade but when I run it the folders, pages and such don't seem to be created. If you've worked it out I'd like to know how.

I contacted PCINET but waiting on a response so thought I'd ask here in case there is some easy thing I might have to tweak. The owner was going to meet and show our site to a bigger customer and I was hoping to get it up and running before then.
